The Caregiver Cliff

The “Caregiver Cliff” describes what happens when individuals with complex disabilities rely on multiple systems—health, PDD, FSCD, housing—but no one is responsible for coordinating them.

Families and caregivers are left to manage everything. When support breaks down, or when a caregiver is no longer able to continue, there is no clear system in place to ensure continuity of care.

Why It Matters

Without coordination, even basic needs like housing, medical care, and daily support become difficult to maintain. Caregivers carry the full burden of navigating systems that don’t work together.

For many, the concern is simple: What happens when I’m no longer here to manage this?
